X-rays may show damage, but pain does not always match the image.
The knee works as part of a chain including muscles, hips, and movement patterns.
Weak hip and leg muscles can change knee mechanics and create pain.
Clinical exercise physiologists analyse the whole movement chain.
Targeted exercise can often reduce pain and improve knee function.
The ESCAPE trial showed exercise greatly reduced the need for surgery.
Only 26% of the exercise group needed surgery compared with 62% in standard care.
Programmes must be personalised and supervised by trained professionals.
Consistency is essential and exercises must become a long-term habit.
Weight management and correct rehabilitation also play key roles.
Exercise may not cure severe damage but can delay or avoid surgery.
